The main shrine is dedicated to the Buddha, in a standing posture known as Phra Si Sakkayamuni. Worshippers at Wat Suthat Thepwararam use the traditional method of bowing three times at the entrance of the temple and then walking clockwise around the shrine three times. They then kneel in front of the image and offer incense and several kinds of flowers accompanied by a small prayer. They will then present a small monetary offering and a folded lotus flower before prostrating themselves nine times, three times each in front, in the middle, and behind the shrine.
