It stands on the site from which the ancient titular church of Santa Maria in Trastevere was demolished in the early Christian period. It is the oldest church dedicated to the Virgin Mary in Rome and may date back to the fourth century.

The first basilica of Our Lady in Trastevere is believed to have been built in the 4th century, possibly on the site of a sacred Roman stream, by Pope Julius I. It was later restored by Pope Gregory I in the 6th century, and then again in the 10th century by Pope John XV. The basilica underwent a major renovation in the 12th century, which included a new façade, bell tower, and several other cosmetic changes.
